Surgical treatment (under general or local anaesthetic)
Surgical treatment may be necessary, especially if the areas of inflammation are extensive or repeatedly inflamed. The procedure involves removing diseased skin tissue under local or general anaesthetic, depending on the extent of the inflammation and the patient’s condition. The aim is to reduce the formation of new abscesses and improve quality of life in a more permanent way. Surgical treatment may also include reconstructive procedures to improve the aesthetic and functional outcome.
CO2 laser deroofing
CO2 laser deroofing is a precise and tissue-sparing procedure that opens up fistula passages that have formed under the skin. Laser treatment reduces inflammation, speeds up healing and can significantly improve patients’ quality of life. The procedure is performed under local anaesthetic and is particularly suitable for recurring, chronic abscesses.
Laser hair removal
Laser hair removal can be used to reduce hair in areas typically affected by HS, such as the underarms and groin. This can reduce the risk of inflammation of the sweat glands and hair follicles and thus prevent the formation of new abscesses. The treatment helps to prevent further inflammation and maintains better skin condition in asymptomatic areas. It is therefore a preventive support measure best suited to mild or moderate HS.
Botulinum injections to control sweat gland activity
Botulinum toxin can be injected under the skin in areas where hyperhidrosis is aggravating the symptoms. Reducing sweat gland activity can help to control recurring inflammation and relieve pain and discomfort associated with sweating. The effect of the treatment is temporary, but lasts for several months and can be repeated if necessary.
Systemic medicines and topical medication
Drug treatment for HS is planned on an individual basis. Topical medication, such as antibiotic ointments, can relieve mild infections. In moderate to severe HS, oral antibiotics, hormonal drugs or biological drugs that affect the immune system are often used. Drug treatment is an important part of the overall treatment and aims to prevent disease progression and reduce inflammation.
